Grievance Process Notice 

At Dirkse CC, we are committed to providing high-quality services and ensuring that all individuals have the right to express concerns about their care. If you are dissatisfied with any aspect of your services, you have the right to file a grievance. 

How to File a Grievance 

You may submit a grievance verbally or in writing to any Dirkse CC staff member. A grievance form is available upon request. Once received, we will acknowledge your grievance within five (5) business days and work to resolve it within thirty (30) calendar days. If additional time is needed, we will inform you of the delay and provide an updated timeline. 

Expedited Review 

If your grievance involves a situation that may cause harm before the standard grievance process is completed, you may request an expedited review. In these cases, the program administrator will review and respond within 48 hours. 

Appeal Process 

If you are not satisfied with the outcome of your grievance, you may file an appeal within ten (10) working days of receiving a decision. Appeals should be submitted in writing to the Oregon Health Authority’s Behavioral Health Division. If requested, Dirkse CC staff will assist you with this process. The Division will provide a written response within ten (10) working days. If you are still not satisfied, you may file a second appeal within ten (10) working days to the Division Director. 

Protection Against Retaliation 

You will not face any form of retaliation for filing a grievance or appeal. Retaliation includes dismissal, reduction in services, harassment, or negative performance reviews based on your complaint. If you experience retaliation, please report it immediately.
